Making your first custom survey

  1. Select the green New Survey button either at the top of the page or the gray New Survey button in the middle of the page.
  2. A dialog opens where you need to enter a survey title in the first input field (required).
  3. For Email, choose Optional, Required, or None.
  4. If you’d like, enter a welcome message in Welcome Message. This message appears at the beginning of your survey for your participants.
  5. Select Save.
  6. On the next page, select New Question.
  7. This page is the question building page. On the left are all the options for your question. In the center of the page is the preview of your question.
  8. In Question Title, enter your question.
  9. If you want the question to be required, select Required.
  10. For Question Type, select a question type: Multiple Choice, Essay, Rating, Scale, Form, Scoring, Range, Date/Time, or Image Choice.
  11. Based on the question type you choose, the available options change. Enter any requirements or answers your question type needs.
  12. If you would like a subtitle to your question, select Advanced Options and, in Subtitle, enter the subtitle.
  13. Select Save Question above the Question Preview to save your question to your survey.
